Job Description

American Traveler is seeking an experienced Home Health RN Case Manager for a day shift position in Vermont, requiring OASIS and case management experience.

Responsibilities

• Home health setting providing patient care in the community
• Day shifts scheduled Tuesday through Saturday
• Assignment duration of 20 weeks
• Responsible for performing comprehensive patient assessments and managing individualized care plans
• No requested time off permitted during the contract
• Local and travel candidates may be considered without a radius restriction

Requirements

• Active VT or compact RN license required
• Minimum of 1 year of home health RN experience
• Current BLS/BCLS certification required
• OASIS assessment experience required
• Case management experience in the home health setting required
• Strongly prefer candidates with prior travel assignment experience
